Formula and scoring

Age 65–74 2, ≥75 3; DM/HTN/angina 1; SBP <100 3; HR >100 2; Killip II–IV 2; weight <67 kg 1; anterior STE or LBBB 1; time to treatment >4 h 1. Range 0–14.

Exam and ward pearls

  • Low systolic BP (3 points) and age ≥75 (3 points) carry the most weight.

When not to rely on it

  • Derived in fibrinolysis-eligible patients (InTIME II, n = 14,114); calibration differs after primary PCI.
