DAPT score

Also called DAPT risk score, prolonged dual antiplatelet therapy score

Whether continuing DAPT beyond 12 months after PCI is likely to give more ischaemic benefit than bleeding harm.

Age

Formula and scoring

Age 65–<75 −1, ≥75 −2; smoking, diabetes, MI at presentation, prior PCI/MI, paclitaxel stent, stent <3 mm 1 each; CHF/LVEF <30% 2; vein graft PCI 2. Range −2 to 10; ≥2 favours prolonged DAPT.

Exam and ward pearls

  • Age lowers the score: older patients bleed more and gain less from prolonged DAPT.

When not to rely on it

  • Apply only at 12 months to patients who completed DAPT without major bleeding or ischaemic events.
  • Modest discrimination (c about 0.64–0.70); the authors call for prospective evaluation.
