CISNE (Clinical Index of Stable Febrile Neutropenia)

Also called CISNE score

Predicts serious complications in solid-tumour patients with apparently stable febrile neutropenia.

CISNE score

0 / 8

Class
Low risk (class I)
Serious complications (validation)
1.1%

Low risk (class I). FINITE validation (1,133 patients, 25 Spanish hospitals): complications in 1.1%, 6.2% and 36% of low, intermediate and high classes. CISNE discriminated better than MASCC in this population (AUC 0.868 vs 0.721).

Formula and scoring

ECOG ≥2: 2; stress hyperglycaemia: 2; COPD: 1; chronic cardiovascular disease: 1; mucositis grade ≥2: 1; monocytes <200/µL: 1. 0 low, 1–2 intermediate, ≥3 high.

Exam and ward pearls

  • Use CISNE only after confirming the patient is clinically stable (no organ dysfunction, normal vital signs, no major infection) — that is where MASCC is weakest.

When not to rely on it

  • Solid tumours only; validated in Spanish outpatients on conventional chemotherapy.
