Carboplatin dose — Calvert formula
Also called Calvert formula, carboplatin AUC dosing, AUC dosing
Carboplatin dose from target AUC and GFR, with a Cockcroft–Gault GFR option and the FDA 125 mL/min cap.

Formula and scoring
Dose (mg) = target AUC × (GFR + 25). Cockcroft–Gault: CrCl (mL/min) = (140 − age) × weight (kg) × [0.85 if female] ÷ (72 × SCr mg/dL). FDA: cap estimated GFR at 125 mL/min → maximum dose = AUC × 150 mg.
Exam and ward pearls
- The "+25" is non-renal clearance; Calvert’s first formula was AUC × (1.2 × GFR + 20).
- Maximum doses with the cap: AUC 6 → 900 mg, AUC 5 → 750 mg, AUC 4 → 600 mg.
When not to rely on it
- Calvert derived the formula with measured GFR. Cockcroft–Gault estimates creatinine clearance and is a surrogate, less reliable at extremes of weight and in unstable renal function.
- A very low serum creatinine inflates the estimate; follow your unit’s protocol on minimum creatinine values.
