Absolute neutrophil count (ANC)

Also called ANC, neutropenia grade, neutrophil count calculator

Absolute neutrophil count from total WBC and the segmented-neutrophil and band percentages, with neutropenia severity and CTCAE grade.

×10⁹/L is the same as ×10³/µL.

Formula and scoring

ANC (/µL) = WBC (/µL) × (segmented % + band %) ÷ 100. Severity: mild 1000–<1500, moderate 500–<1000, severe <500, very severe <200 /µL. CTCAE v5.0: grade 2 <1500–1000, grade 3 <1000–500, grade 4 <500.

Exam and ward pearls

  • Lower limit of normal is 1500/µL in adults and older children but 1000/µL up to 12 months of age.
  • Include bands: leaving them out underestimates ANC in left-shifted counts.

When not to rely on it

  • Infant normal range differs (LLN 1000/µL up to 12 months), so the adult bands overstate severity in infants.
  • Boundaries follow CTCAE: exactly 1000/µL is mild (grade 2) and exactly 500/µL is moderate (grade 3).
