Must document for this operation
- Gestational age by ultrasound and the legal ground for termination. The MTP Act ties the number of opinions and the forms needed to gestational age, and the ground must match what is written on the opinion form.
- Admission Register serial number and which opinion form was used. The opinion form and the register are linked by this number, so the note can be traced without repeating the woman's identity.
- Method (MVA or EVA) and cannula size. The Admission Register has a column for method of MTP, and cannula size shows the evacuation matched the gestation.
- What was seen in the aspirate: villi, sac or fetal parts. If no villi are seen, ectopic pregnancy or failed abortion must be ruled out, and the next doctor needs to know this.
- Any suspected perforation and what was done. A perforation with a suction cannula needs assessment for bowel injury, and the record protects both patient and surgeon.
- Rh status and anti-D given or not. An Rh-negative woman who misses anti-D can be sensitised for her next pregnancy.
- Post-abortion contraception chosen. It has its own column in the Admission Register, and an IUCD batch number is needed if the device fails or is recalled.
Common mistakes in these notes
- Writing the woman's name and address in notes or registers that go outside the hospital; the MTP Act forbids revealing her identity except to persons authorised by law.
- Writing 'S&E done' with no method, cannula size or end-point.
- Not recording whether villi were seen in the aspirate.
- Missing the time of misoprostol and antibiotic given before the procedure.
- Forgetting anti-D for an Rh-negative woman, or not writing that she is Rh positive.
- Doing a tubal ligation with the MTP without a separate sterilisation consent form.
Indian practice and paperwork
- Consent for termination is Form C (rule 9 of the MTP Rules). Only the woman signs; a guardian signs only if she is a minor or a mentally ill person. (MTP Rules Form C, updated formats circulated by Commissionerate of Health, Gujarat, 1 Jan 2022: https://nhm.gujarat.gov.in/images/pdf/MTP-Amendements-Rules-2021.pdf)
- Up to 20 weeks, the opinion of one registered medical practitioner is recorded in Form I 'RMP Opinion Form (For gestation age upto twenty weeks)' under Regulation 3, which also carries the Admission Register serial number. Form E (two RMPs) is for over 20 up to 24 weeks. (Gujarat circular above; MTP (Amendment) Rules 2021, G.S.R. 730(E), rule 4A: https://egazette.gov.in/WriteReadData/2021/230390.pdf)
- For termination up to 12 weeks by a surgical method, the opinion can be given by an RMP eligible under clauses (a), (b), (c) and (d) of rule 4. (MTP (Amendment) Rules 2021, rule 4A(1)(b): https://egazette.gov.in/WriteReadData/2021/230390.pdf)
- Every termination is entered in the Admission Register (Form III, Regulation 5), which has columns for method of MTP (MVA/EVA/MMA/D&C/Others) and post-abortion contraception (TL/IUCD/OCP/Injectables/Others/None). The register is destroyed five years after the last entry. (Gujarat circular above (Form III, 16 columns))
- Section 5A: an RMP must not reveal the name and other particulars of a woman whose pregnancy was terminated, except to a person authorised by law. Breach is punishable with up to one year imprisonment, fine, or both. (MTP (Amendment) Act 2021, s.5A: https://prsindia.org/files/bills_acts/acts_parliament/2021/Medical%20Termination%20of%20Pregnancy%20Amendment%20Act%202021.pdf)
- Fetus below the viability period is yellow category (a) human anatomical waste, handed to the common bio-medical waste facility in a yellow bag with a copy of the official MTP certificate. (DGHS/CPCB guidelines under BMW Rules 2016)
- Tubectomy can be done concurrently with surgical MTP or within 7 days after it, on the separate sterilisation consent (Annexure 1). (MoHFW Standards & Quality Assurance in Sterilization Services (2014), section 2.2: https://nhm.gov.in/images/pdf/programmes/family-planing/guidelines/Standards_and_Quality_Assurance.pdf)
