Worked example
Fictional case: Mrs. A.B., 27 years, G2P1 at 39+0 weeks, spontaneous vaginal delivery with active management of third stage, placenta not delivered 45 minutes after birth with ongoing bleeding of 450 ml.
Case details
Date: 09/06/2026. Manual removal of placenta. OT 2.
Baby born 14:05. Decision for MRP 14:40. In OT 14:50. Spinal 14:55. Placenta out 15:05. Procedure ended 15:15.
Surgeon: Dr A. (2nd-year resident, OBGY). Assistant: Dr B. Consultant Dr C. informed.
Anaesthetist: Dr D. Scrub nurse: Sr E.
Indication and pre-op
Indication: retained placenta, 45 min after birth, with vaginal bleeding of 450 ml.
Inj oxytocin 10 IU IM at 14:05. Second dose oxytocin 10 IU IM at 14:35 with controlled cord traction: placenta did not come. Bladder emptied by catheter.
No previous caesarean or uterine surgery. Placenta fundal on last scan.
Pulse 104/min, BP 110/70 mmHg. Hb 10.4 g/dl before labour. Blood group A positive, 2 units cross-matched. 18G IV line x 2.
Consent for MRP, blood transfusion and possible laparotomy taken in Hindi from patient, husband as witness. WHO checklist done.
Anaesthesia and position
Spinal anaesthesia. Lithotomy. Perineum cleaned with povidone-iodine and draped. Bladder emptied with in-out catheter. Oxytocin infusion 20 IU in 1000 ml RL kept ready.
Inj cefazolin 1 g IV at 14:57, single dose.
Procedure
Right hand in elbow-length glove, lubricated, passed in a cone shape through the vagina and cervix along the cord. Left hand held the fundus through the abdomen throughout.
Cervix partly closed, opened with steady gentle pressure. Placenta adherent at the posterior fundus. Edge found and a plane developed with the finger tips. Placenta separated completely and removed in the palm at 15:05.
Cavity explored again: empty, walls intact.
Oxytocin infusion started at 150 ml/h. Bimanual massage given. Uterus contracted well.
Placenta and membranes checked: complete, no missing cotyledon, no vessel running to the membrane edge.
Closure
Cervix and vagina inspected: no tears. Old first degree tear not bleeding. No vaginal pack. Swab count correct.
End of procedure
Estimated blood loss 650 ml in total (450 ml before + 200 ml during MRP, measured in drape and weighed pads). No transfusion.
No intra-operative complications.
Specimen: placenta not sent for HPE. Disposed in yellow bag as anatomical waste.
Shifted to postnatal ward. Pulse 96/min, BP 112/72 mmHg. Uterus well contracted.
Post-op orders
Pulse, BP, uterine tone and PV bleeding every 15 min for 1 hour, then every 30 min for 4 hours.
Oxytocin infusion to complete.
Foley catheter to be removed when sensation returns; record urine output.
Hb after 12 hours.
Temperature 6-hourly. Inform if fever or foul-smelling lochia.
Explained to patient that the placenta had to be removed by hand, and that she should deliver next time in a hospital.

Must document for this operation
- Time of birth, time of decision and time placenta was removed. The length of the third stage and any delay in acting are reviewed in PPH audits.
- Uterotonics given before MRP (manual removal of placenta), with dose and time. Shows that the third stage was actively managed before going to theatre.
- Whether the placenta separated easily, and any area that would not separate. A placenta that would not separate raises suspicion of accreta, which matters now and in every later pregnancy.
- Completeness of placenta and membranes and state of the cavity after removal. If she bleeds later, the next doctor needs to know whether retained tissue is likely.
- Antibiotic given. MRP carries a risk of endometritis and a single prophylactic dose is expected.
- Scar intact on exploration (if previous caesarean). The hand can go through a thin scar, and a scar defect changes the plan.
Common mistakes in these notes
- Writing 'MRP done' with no note on whether the placenta was complete.
- Not recording the time the baby was born, so the length of the third stage is unknown.
- Leaving out the blood loss before theatre and counting only the loss during MRP.
- Not recording that the fundus was held from above during removal.
- Not stating what was done with the placenta.
Indian practice and paperwork
- Placenta is listed as human anatomical waste (Yellow category) under the Bio-Medical Waste Management Rules 2016. Record whether it was sent for HPE, discarded in the yellow bag, or handed over. (DGHS/CPCB Guidelines for Management of Healthcare Waste as per BMW Rules 2016)
