Formula and scoring

SCORAD = A/5 + 7B/2 + C. A = % BSA (rule of nines, 0–100); B = sum of six intensity items (0–3 each, 0–18); C = itch VAS + sleep-loss VAS (0–20). Objective SCORAD = A/5 + 7B/2 (0–83).

Exam and ward pearls

  • Intensity (B) carries about 60% of the weight; extent and subjective symptoms about 20% each.
  • Rule of nines for extent in SCORAD: head/neck 9, each arm 9, each leg 18, front trunk 18, back 18, genitals 1.

When not to rely on it

  • Severity strata were validated in patients aged 13 and older; older bands (<25 / 25–50 / >50) are often quoted but differ.
  • The subjective part (C) is patient-reported, so SCORAD can be >0 in clear skin.
