Reticulocyte index and reticulocyte production index

Also called RPI, corrected reticulocyte count, reticulocyte index, CRC

Formula and scoring

Reticulocyte index (corrected %) = retic % × patient Hct ÷ normal Hct (45). RPI = reticulocyte index ÷ maturation time. Maturation time by Hct: ≥40% = 1.0, 30–39.9% = 1.5, 20–29.9% = 2.0, <20% = 2.5 days. RPI >3 adequate, <2 inadequate.

Exam and ward pearls

  • Two corrections: Hct corrects for fewer red cells; maturation time corrects for stress reticulocytes released early that circulate longer.
  • A normal person without anaemia has an RPI of about 1.

When not to rely on it

  • Maturation-time bands differ slightly between textbooks; this page uses the MSD Manual table (from Henry, Clinical Diagnosis).
  • Use the absolute reticulocyte count from automated analysers where available.
