Formula and scoring
Urea (mmol/L) 6.5–7.9 = 2, 8.0–9.9 = 3, 10.0–24.9 = 4, ≥25 = 6. Hb men 12.0–12.9 = 1, 10.0–11.9 = 3, <10 = 6; women 10.0–11.9 = 1, <10 = 6. SBP 100–109 = 1, 90–99 = 2, <90 = 3. Pulse ≥100 = 1, melaena = 1, syncope = 2, hepatic disease = 2, cardiac failure = 2.
Exam and ward pearls
- Uses no endoscopic data, so it is the triage score in the emergency department.
- GBS predicts need for intervention best; AIMS65 and full Rockall predict death better (Stanley 2017).
When not to rely on it
- Urea is in mmol/L; convert BUN in mg/dL with the unit switch (BUN ÷ 2.8).
