BISAP score
Also called Bedside Index of Severity in Acute Pancreatitis
Five bedside variables in the first 24 hours that predict in-hospital mortality in acute pancreatitis.
BISAP
0 / 5
Mortality rose with each point, from under 1% at the lowest scores to over 20% at the highest (US derivation, 17,992 cases; AUC 0.82 in validation, similar to APACHE II).
Formula and scoring
One point each within 24 h: BUN > 25 mg/dL, impaired mental status, SIRS, age > 60, pleural effusion.
Exam and ward pearls
- Computable at admission, unlike Ranson (needs 48 h).
- BUN 25 mg/dL ≈ urea 8.9 mmol/L (BUN × 0.357).
When not to rely on it
- Predicts death, not local complications; score within the first 24 hours only.
