Formula and scoring

Sum of 7 items, each 0 (not at all) to 3 (nearly every day). Cut points 5, 10 and 15 = mild, moderate, severe. ≥10 = probable GAD.

Exam and ward pearls

  • Cut-off 10: sensitivity 89%, specificity 82% for GAD (Spitzer 2006).
  • The first two items form the GAD-2 screen.

When not to rely on it

  • A screen, not a diagnosis. Validated in US primary care; use a validated translation where needed.
