Formula and scoring
Sum of 11 items (maximum 48). 5–12 mild, 13–24 moderate, 25–36 moderately severe, >36 severe.
Exam and ward pearls
- Buprenorphine, a partial µ-agonist, can precipitate withdrawal in a highly dependent patient who is not yet in withdrawal; COWS documents that withdrawal has begun (Wesson and Ling 2003).
- Only some items allow every number: e.g. restlessness scores 0, 1, 3 or 5 and gooseflesh 0, 3 or 5.
When not to rely on it
- Rate each item with the anchor wording on the official form. Fever, anxiety, stimulant use or other illness can raise pulse, sweating and restlessness.
- The score needed before a first buprenorphine dose is set by local protocol, not by the scale.
