Formula and scoring

eGFR = 142 × min(Scr/κ, 1)^α × max(Scr/κ, 1)^−1.200 × 0.9938^age × 1.012 [female]; κ 0.7 (F) / 0.9 (M); α −0.241 (F) / −0.302 (M).

Exam and ward pearls

  • Race coefficient was removed in 2021; the new equation slightly underestimates GFR in Black and overestimates it in non-Black adults.
  • Creatinine–cystatin C equation is more accurate and is the confirmatory test.

When not to rely on it

  • Assumes steady-state creatinine, so not for AKI. Developed in ambulatory adults without serious illness (Inker 2021).
  • Racial and ethnic groups other than Black and White, including South Asians, were poorly represented.
