Worked example

Fictional case: Mr. S.K., 19 years, 2.5 cm painless midline neck swelling just below the hyoid for 1 year, moving with swallowing and tongue protrusion, one episode of infection treated with antibiotics 4 months ago.

Case details

Date: 08/04/2026. Elective. OT 3.

In OT 09:05. Induction 09:15. Skin incision 09:32. Closure completed 10:40.

Surgeon: Dr A. (3rd-year resident, General Surgery). Assistant: Dr B. (1st-year resident). Consultant: Dr C., scrubbed for hyoid division.

Anaesthetist: Dr D. Scrub nurse: Sr E.

Indication and pre-op

Indication: thyroglossal duct cyst with one previous episode of infection, now quiet for 4 months.

USG neck: 2.4 x 2.0 cm thin-walled cyst just below the hyoid in midline. Thyroid gland normal in size and position.

TSH normal. Hb 14.1 g/dl.

Consent: general, procedure-specific and anaesthesia consent taken in Hindi. Removal of middle of hyoid, recurrence and neck haematoma explained.

WHO checklist sign in and time out done. Inj cefazolin 2 g IV at 09:18, before incision, because of previous infection.

Anaesthesia and position

GA with oral endotracheal tube. Supine, shoulder roll, neck extended, chin in midline. Lower face and lips in the field.

Parts painted with povidone-iodine and draped. 8 ml of 1% lignocaine with adrenaline 1:200,000 infiltrated along the incision.

Procedure

5 cm transverse incision in a skin crease over the cyst. No sinus or scar.

Subplatysmal flaps raised. Deep fascia opened in midline and sternohyoid muscles retracted laterally.

Findings: 2.5 cm smooth cyst below the hyoid, clear mucoid content. No pyramidal lobe. A firm cord ran up from the cyst to the middle of the hyoid body and passed behind it.

Cyst dissected from below upwards, kept intact.

Mylohyoid and geniohyoid detached from upper border and sternohyoid from lower border of hyoid. Hyoid cut on each side of midline, medial to lesser cornua, with heavy scissors. About 1.5 cm of hyoid body removed in continuity with cyst.

With Dr C.'s double-gloved fingers in the mouth pressing on the tongue base, a core of muscle around the tract was cut out up to just below the foramen caecum. Tract base suture ligated with polyglactin 910 (Vicryl) 3-0. Vallecula not entered.

Wound washed with saline. Haemostasis with bipolar cautery.

Closure

Sternohyoid sutured to mylohyoid over the hyoid gap with interrupted Vicryl 3-0.

10 Fr suction drain placed, brought out through a separate stab below the incision.

Strap muscles and platysma closed with Vicryl 3-0. Skin closed with subcuticular poliglecaprone 25 (Monocryl) 4-0.

Specimens

Cyst with tract, middle of hyoid and tongue-base core sent in one piece for HPE in formalin. Superior end marked with a long stitch.

End of procedure

Estimated blood loss 40 ml. No transfusion.

Mop, needle and instrument count correct, confirmed by scrub nurse.

No intra-operative complications. Extubated smoothly, no stridor.

Shifted to recovery in stable condition. Pulse 84/min, BP 124/76 mmHg, SpO2 99% on room air.

Post-op orders

Overnight stay. Watch neck for swelling and breathing through the night. Call surgeon at once if neck swells or stridor.

Drain output charted. Remove when under 20 ml in 24 hours.

Sips when fully awake and no nausea, then soft diet.

Tab paracetamol 1 g 6-hourly.

No further antibiotic (airway not entered).

Review HPE in clinic at 10 days.

Must document for this operation

  • That the middle of the hyoid bone was removed with the cyst, and how much. Simple excision without the hyoid body has a high recurrence rate, so an auditor or the next surgeon will look for this.
  • How far up the tract was followed and how its base was secured. If the cyst comes back, the next surgeon plans a wider excision based on what was left.
  • Pre-operative confirmation of a normal thyroid gland (ultrasound finding). If the swelling was the only thyroid tissue, removing it causes lifelong hypothyroidism, and the note must show this was checked.
  • Whether the airway or vallecula was entered and how it was repaired. The ward team needs to watch for surgical emphysema and continue antibiotics if it was entered.
  • Drain type and the neck-watch order. A neck haematoma can block the airway, and nurses need a written instruction on what to watch and whom to call.
  • Any sinus or scar excised with the specimen. The pathologist and the next surgeon need to know the specimen included skin from a previous drainage.

Common mistakes in these notes

  • Writing 'cyst excised' with no mention of the hyoid bone.
  • Not recording the pre-operative ultrasound or thyroid function (TSH) result.
  • Leaving out whether the cyst ruptured during dissection.
  • Sending the specimen without marking which end is superior.
  • No written instruction for the nurses on airway and neck swelling watch.

Indian practice and paperwork

  • NABH standard COP.14(e) (5th edition) requires the operative note to be written before the patient is transferred out of recovery. (NABH Hospital Accreditation Standards 5th ed. (April 2020), COP.14 OE e)
  • Bio-Medical Waste Management Rules 2016 put excised human tissue in Yellow category (a) human anatomical waste, and discarded formalin is chemical waste. Write where the specimen went: sent for HPE in formalin, or discarded as Yellow (a) waste. (DGHS/CPCB Guidelines for Management of Healthcare Waste as per BMW Rules 2016)
  • Name every anaesthetist in the note. An Indian consumer commission held a hospital negligent when the operation note left out the anaesthetist's name. (Thomas J. Indian J Urol 2009;25(3):384-8, PMID 19881136)
