Worked example

Fictional case: Mrs. S.D., 54 years, underwent laparoscopic low anterior resection with a diverting loop ileostomy 14 weeks ago for rectal carcinoma. Water-soluble contrast enema showed a healed anastomosis with no leak or stricture. Admitted for elective closure.

Case details

Date: 18/08/2026. Elective closure of loop ileostomy. OT 1.

In OT 09:05. Induction 09:15. Skin incision 09:32. Closure completed 10:40.

Surgeon: Dr A. (3rd-year resident, General Surgery). Assistant: Dr B. (2nd-year resident). Consultant: Dr C., scrubbed.

Anaesthetist: Dr D. Scrub nurse: Sr E.

Indication and pre-op

Indication: closure of temporary loop ileostomy formed 14 weeks ago to protect a colorectal anastomosis.

Contrast enema 04/08/2026: anastomosis healed, no leak, no stricture. Digital rectal examination: good sphincter tone, anastomosis felt, smooth.

Clear liquids for 24 hours before surgery. Hb 11.8 g/dl, albumin 3.8 g/dl.

Consent: general, procedure-specific and anaesthesia consent taken in Hindi from patient, husband as witness. Told that a laparotomy may be needed if closure through the stoma site is not safe.

WHO checklist sign in and time out done. Inj cefazolin 1 g IV at 09:18, before skin incision.

Anaesthesia and position

General anaesthesia with endotracheal intubation. Supine. NG tube and Foley catheter in situ. Stoma plugged with a swab. Parts painted with povidone-iodine and draped.

Procedure

Circumstomal skin incision 2 to 3 mm outside the mucocutaneous junction. Stoma edge held up with four artery forceps.

Ileum dissected off subcutaneous fat and anterior rectus sheath with scissors, staying on the serosa. Peritoneum entered. Filmy adhesions around the fascial ring freed with a finger.

Findings: healthy loop ileostomy, no parastomal hernia. About 8 cm of each limb mobilised without tension. One small serosal tear on the distal limb, repaired with a single Vicryl 3-0 seromuscular stitch. No full-thickness injury on saline irrigation of each limb.

Eversion reversed. Skin and scarred mucocutaneous edge trimmed back to soft, bleeding bowel.

Enterotomy closed transversely in a single layer of interrupted polyglactin 910 (Vicryl) 3-0, corner stitches first, then middle, sutures about 4 mm apart, sparing the mucosa.

Lumen admits a thumb. No leak. Bowel returned to the abdomen.

Closure

Rectus sheath closed with interrupted polypropylene (Prolene) 1-0, sutures placed first and then tied.

Skin closed with a subcuticular purse-string, leaving a central opening of about 1 cm for drainage. No drain. Gauze dressing.

End of procedure

Estimated blood loss 40 ml. No transfusion.

Mop, needle and instrument count correct, confirmed by scrub nurse.

No intra-operative complications apart from the serosal tear noted above.

Specimens: trimmed stoma edge, discarded.

Patient extubated and shifted to post-operative ward in stable condition. Pulse 84/min, BP 122/76 mmHg.

Post-op orders

Pulse, BP, temperature 4-hourly. Abdominal examination twice daily.

NG tube on free drainage, remove at 48 hours if aspirate is low.

Oral liquids from 48 hours, then low-residue diet.

IV fluids and analgesia as per chart.

Wound inspected daily. Inform if faecal discharge from wound.

Chart first flatus and first stool. Catheter out on POD 1 if mobile.

Perianal skin care. Review in surgical OPD 2 weeks after discharge.

Must document for this operation

  • How the distal bowel and anastomosis were assessed before closure, with the result. Closing a stoma above an unhealed anastomosis or a distal stricture causes a leak, and the note must show this was checked.
  • Any serosal tear or enterotomy made during mobilisation and how it was repaired. If a fistula appears later, the team needs to know whether the bowel was injured at the operation.
  • Method of closure: transverse hand-sewn, stapled side-to-side, or resection, with suture or stapler details. This guides management of a leak or stricture and is audited against outcomes.
  • How the fascia was closed. Incisional hernia is common at old stoma sites, and a later hernia repair starts from this record.
  • Whether the skin was closed, purse-stringed or left open. Nurses plan dressings from it, and wound infection rates differ between methods.
  • Any extension of the incision or conversion to laparotomy, with the reason. It changes recovery and is a common consent question afterwards.

Common mistakes in these notes

  • Writing 'ileostomy closed' without the method of bowel closure.
  • Not recording the contrast enema or endoscopy result in the pre-op section.
  • Leaving out a serosal tear that was repaired.
  • No mention of fascial closure technique.
  • Not stating whether the skin was left open.

Indian practice and paperwork

  • The ICMR antimicrobial guideline lists a single dose of cefazolin, ampicillin-sulbactam or ceftriaxone for clean-contaminated operations, first dose within 1 hour before incision, not continued beyond 24 hours.
