Indication and pre-op
Indication: suspected malignant right ovarian tumour, apparent early stage on CT.
CT chest normal. Upper GI endoscopy and colonoscopy normal. Family complete, agrees to hysterectomy and removal of both ovaries.
Blood group O positive, 2 units cross-matched. Frozen section booked with pathology.
Consent: general, procedure-specific and anaesthesia consent taken in Hindi from the patient, son as witness. Consent covers staging, lymph node removal, appendicectomy and possible bowel surgery.
WHO checklist sign in and time out done. Inj cefazolin 2 g IV at 08:55, before incision.
Anaesthesia and position
General anaesthesia with thoracic epidural. Supine. Foley catheter in situ. Abdomen painted with chlorhexidine-alcohol and draped.
Procedure
Vertical midline incision from pubis to 4 cm above the umbilicus. Self-retaining retractor placed.
No ascites. 100 ml normal saline instilled and aspirated from pelvis, both paracolic gutters and both subdiaphragmatic spaces, sent for cytology.
Findings: right ovary replaced by an 11 x 9 cm mass, solid and cystic, capsule intact, no surface growth, not adherent. Left ovary, both tubes and uterus normal. Both hemidiaphragms, liver, gallbladder, stomach, spleen and omentum normal. Small bowel run from ileocaecal junction to DJ flexure, colon and appendix normal. No peritoneal deposits. No enlarged pelvic or para-aortic nodes on palpation.
Right ureter identified. Right infundibulopelvic ligament clamped, cut and ligated. Right salpingo-oophorectomy done, mass removed intact without spill, sent for frozen section.
Frozen section (10:25): high-grade serous carcinoma.
Total abdominal hysterectomy with left salpingo-oophorectomy done in the standard way, ureters seen on both sides.
Infracolic omentectomy: omentum lifted, plane between omentum and transverse colon opened, lesser sac entered, dissection taken from hepatic to splenic flexure. Pedicles tied with polyglactin 910 (Vicryl) 2-0. Gastroepiploic arcade kept. No traction on the spleen.
Peritoneal biopsies taken from pouch of Douglas, bladder peritoneum, both pelvic side walls, both paracolic gutters and both hemidiaphragms.
Bilateral pelvic lymphadenectomy from mid common iliac to circumflex iliac vein, including obturator fossa above the obturator nerve. Obturator nerves safe.
Bilateral para-aortic lymphadenectomy from mid common iliac arteries up to the left renal vein.
Appendix normal, not removed (serous histology).
No gross residual disease. Haemostasis checked. Both ureters peristalsing. Warm saline wash given.
Closure
One closed suction drain in the pelvis through the left flank.
Sheath closed with continuous polydioxanone (PDS) No. 1, small bites 5 to 8 mm apart. Subcutaneous tissue not sutured. Skin closed with staples. Sterile dressing.
Specimens
1. Peritoneal washings for cytology.
2. Right ovarian mass with tube (frozen section done).
3. Uterus with cervix, left tube and ovary.
4. Infracolic omentum.
5 to 12. Peritoneal biopsies, 8 sites, each labelled.
13 to 16. Right pelvic, left pelvic, right para-aortic (paracaval) and left para-aortic nodes, separately.
All in 10% formalin except washings. Requisition with CA-125 and imaging details.
End of procedure
Estimated blood loss 750 ml. 1 unit PRBC transfused (Hb 9.6 g/dl pre-op).
Mop, needle and instrument count correct, confirmed by scrub nurse.
No intra-operative complications. No bowel, bladder, ureter or vessel injury.
Provisional surgical stage: FIGO I A (right ovary only, capsule intact), pending final HPE and cytology.
Shifted to HDU in stable condition. Pulse 92/min, BP 124/78 mmHg, urine 60 ml/h.
Post-op orders
Pulse, BP, SpO2 and urine output hourly for 24 hours. Drain output 8-hourly.
Nil by mouth today. Sips on POD 1 if bowel sounds present, then liquids.
Epidural analgesia as per anaesthetist.
Stockings and calf pump. LMWH as per unit protocol. Mobilise from POD 1.
Hb and serum electrolytes on POD 1. Catheter out when epidural is removed.

Must document for this operation
- Whether the capsule was intact, ruptured before surgery, or ruptured during surgery. This alone moves the stage between I A and I C and changes the need for chemotherapy.
- Site and size of every deposit seen, including a clear statement of areas that were normal. Stage and prognosis depend on where and how big the disease was, and this information is lost if it is not written at the time.
- Volume of ascites or that washings were taken. Positive cytology changes the stage, and the pathologist needs to know which sample is which.
- Frozen section result with the time it was reported. It justifies the extent of surgery, especially removing the uterus and other ovary.
- Each staging component done or not done, with the reason for any omission. Incomplete staging is a common error and decides whether a second operation or chemotherapy is needed.
- Upper limit of the para-aortic dissection and the node groups sent separately. Para-aortic nodes can be positive with negative pelvic nodes, so the oncologist must know which basins were sampled.
- Residual disease at the end, with the largest size. Residual tumour size is a main predictor of survival and is recorded for the oncology team.
Common mistakes in these notes
- Writing 'TAH BSO done' with no mention of washings, biopsies, omentum or nodes.
- Not stating whether the tumour was removed intact.
- Sending all peritoneal biopsies or node groups in one container.
- Writing 'upper abdomen normal' without listing what was examined.
- Leaving out the provisional stage and the plan to confirm it on HPE.
