Worked example

Fictional case: Mr. G.S., 38 years, appendicectomy 6 years ago, 2 days of colicky abdominal pain, bilious vomiting and no flatus, with constant pain and tenderness in the right lower abdomen since 10 hours, pulse 110/min.

Case details

Date: 03/11/2026. Emergency. Emergency OT.

Admitted 14:20. Decision for surgery 17:00. In OT 18:10. Induction 18:20. Incision 18:35. Closure completed 20:30.

Surgeon: Dr A. (3rd-year resident, General Surgery). Assistant: Dr B. (2nd-year resident). Consultant: Dr C., present in OT.

Anaesthetist: Dr D. Scrub nurse: Sr E.

Indication and pre-op

Indication: acute small bowel obstruction with features of strangulation (constant pain, localised tenderness, tachycardia, lactate 3.8 mmol/l). CECT: closed-loop obstruction in the right lower quadrant with poor wall enhancement.

Hb 15.1 g/dl, TLC 15,200/cumm, urea 52 mg/dl, creatinine 1.2 mg/dl, sodium 134, potassium 3.4 mmol/l.

Resuscitated with 2.5 L RL. NG tube 16 Fr: 900 ml bilious aspirate. Foley catheter: 40 ml/h urine.

Stoma site marked in the right iliac fossa in case needed.

Consent: general, procedure-specific, high-risk and anaesthesia consent taken in Hindi from patient and brother. Resection, possible stoma and leak explained.

WHO checklist sign in and time out done. Inj cefazolin 2 g IV and inj metronidazole 500 mg IV at 18:20, before incision.

Anaesthesia and position

GA with rapid sequence intubation. Supine. Parts painted with chlorhexidine-alcohol and draped.

Findings

About 300 ml blood-stained, non-foul peritoneal fluid.

A single fibrous band from the old appendicectomy site to the ileal mesentery, trapping a loop of ileum.

Trapped segment 40 cm long, starting 60 cm from the ileocaecal junction. Dark purple to black, no sheen, no peristalsis, no mesenteric pulsation. Not perforated.

Proximal small bowel dilated up to 4.5 cm. Distal ileum collapsed. Caecum collapsed.

Whole small bowel run from DJ flexure to ileocaecal junction: no other ischaemia. Colon normal. Hernial orifices normal.

Procedure

Midline incision of 18 cm, centred on the umbilicus. Peritoneal fluid sucked out.

Band divided between ligatures. Trapped loop released and covered with warm moist mops for 10 minutes with 100% oxygen: no recovery of colour or peristalsis. Decided to resect.

Bowel contents milked back to the NG tube.

Resection margins marked in healthy pink bowel beyond the dark segment at each end. Mesentery divided in a V shape, vessels ligated with silk 3-0.

Bowel divided between clamps. Total 50 cm of ileum resected.

Both cut ends pink with bright red bleeding from the cut edge. Mesenteric fat cleared for about 3 mm at each end.

Diameter mismatch: Cheatle cut of 2 cm on the antimesenteric border of the distal collapsed end.

End-to-end hand-sewn anastomosis, single layer of interrupted extramucosal polyglactin 910 (Vicryl) 3-0 sutures, about 5 mm apart and 5 mm from the edge. Mesenteric and antimesenteric stay sutures first, front half, then bowel turned over for the back half.

Anastomosis checked: patent between finger and thumb, no tension, no twist, ends pink.

Mesenteric defect closed with interrupted Vicryl 3-0, peritoneum only.

Remaining small bowel measured along the antimesenteric border: about 450 cm.

Peritoneal lavage with 3 L warm normal saline. Omentum placed over the anastomosis.

Closure

No drain.

Linea alba closed with continuous looped polydioxanone (PDS) No. 1, mass closure.

Skin closed with interrupted polypropylene (Prolene) sutures. Dressing applied.

Specimens

Resected ileum, 50 cm, proximal end marked with a silk stitch, in formalin, sent for HPE.

Peritoneal fluid for culture and sensitivity sent.

End of procedure

Estimated blood loss 200 ml. No transfusion.

Mop, needle and instrument count correct, confirmed by scrub nurse.

No intra-operative complications.

Extubated in OT. Shifted to surgical HDU. Pulse 96/min, BP 122/78 mmHg, SpO2 98% on 2 L oxygen.

Post-op orders

HDU care. Pulse, BP, SpO2 and urine output hourly for 12 hours.

Nil by mouth. NG tube on free drainage with 4-hourly aspiration.

IV RL and NS with potassium as per charted volumes. Electrolytes and creatinine daily.

No further antibiotic after the pre-operative dose (no perforation, no pus). Review with culture report.

Inj paracetamol 1 g IV over 15 min 6-hourly.

Stockings. Incentive spirometry. Mobilise from POD 1.

Oral sips after flatus is passed and abdomen is soft. Watch for fever, tachycardia or new pain from POD 3 (leak).

Must document for this operation

  • Length of bowel resected and length of small bowel left. Short bowel problems depend on what remains, and the next surgeon needs the figure before any further resection.
  • Site of the segment (distance from DJ flexure or ileocaecal junction). The site tells the next doctor which part of the bowel is gone, and guides nutrition follow-up.
  • How viability was judged and the time waited. Shows that a doubtful loop was given a chance before resection, which is often questioned in audits.
  • Type of anastomosis, technique and suture material, or stapler details with sticker. If a leak occurs, the note explains what was built, and stapler lots may be needed for a recall.
  • Mesenteric defect closed. An open defect can cause an internal hernia later, and the note shows it was dealt with.
  • The reason for a stoma, if one was made instead of an anastomosis. Needed for the plan and timing of stoma closure and for counselling the family.

Common mistakes in these notes

  • Writing 'R&A done' with no length resected, no site and no technique.
  • Not measuring or recording the length of remaining small bowel after a long resection.
  • Not writing that the whole bowel was run from DJ flexure to ileocaecal junction.
  • Leaving out how viability was assessed.
  • Not recording the stapler make and cartridge, or not pasting the sticker.

Indian practice and paperwork

  • The NQAS district hospital OT checklist expects that braided biological sutures are not used in dirty wounds and that catgut is not used to close the fascia of abdominal wounds. (NQAS Assessor's Guidebook for District Hospitals 2020, Vol I, OT checklist ME E15.3)
  • NQAS expects post-operative notes to record vital signs, pain control, rate and type of IV fluids, urine and GI fluid output, other medicines and lab tests. Write each of these in the post-op orders. (NQAS Assessor's Guidebook for District Hospitals 2020, Vol I, OT checklist ME E15.4)
  • Bio-Medical Waste Management Rules 2016 put excised human tissue in Yellow category (a) human anatomical waste, and discarded formalin is chemical waste. Write where the specimen went: sent for HPE in formalin, given to relatives with a note, or discarded as Yellow (a) waste. (DGHS/CPCB Guidelines for Management of Healthcare Waste as per BMW Rules 2016)
