Worked example
Fictional case: Mr. R.M., 62 years, with iron deficiency anaemia and colonoscopy showing an ulceroproliferative growth in the ascending colon, biopsy adenocarcinoma. CT: 5 cm ascending colon mass, no liver or lung metastases. CEA 6.2 ng/ml.
Case details
Date: 22/06/2026. Elective open radical right hemicolectomy. OT 1.
In OT 08:45. Induction 09:00. Skin incision 09:25. Closure completed 12:05.
Surgeon: Dr C. (Associate Professor). Assistants: Dr A. (3rd-year resident), Dr B. (1st-year resident).
Anaesthetist: Dr D. Scrub nurse: Sr E.
Indication and pre-op
Indication: adenocarcinoma of the ascending colon, clinically non-metastatic.
Colonoscopy complete to caecum, no other lesion. Hb 10.4 g/dl after two units transfused last week. 2 units cross-matched.
Consent: general, procedure-specific and anaesthesia consent taken in Hindi from patient, wife as witness. Possibility of ileostomy explained.
WHO checklist sign in and time out done. Inj piperacillin-tazobactam 4.5 g IV at 09:05, before skin incision.
Anaesthesia and position
General anaesthesia with endotracheal intubation. Supine. Foley catheter and NG tube in situ. Parts painted with povidone-iodine and draped.
Findings
5 x 4 cm growth in the mid ascending colon, mobile, serosa puckered, not adherent to abdominal wall or duodenum.
Liver smooth, no palpable nodules. No ascites, no peritoneal or omental deposits. Pelvis normal.
A few soft nodes along the ileocolic pedicle. No para-aortic nodes. Rest of the colon normal on palpation.
Procedure
Midline incision of about 18 cm centred on the umbilicus. Self-retaining retractor placed.
Medial to lateral approach. Ileum and transverse colon encircled with tapes at the planned lines of division.
Right branch of middle colic artery and vein ligated at origin. Ileocolic artery and vein dissected, identified separately from the SMA and SMV, and each double ligated with silk about 1 cm from the SMA, then divided. No separate right colic artery found.
Right colon and mesocolon lifted off the retroperitoneum from medial to lateral, and the lateral peritoneum along the white line of Toldt divided last. Right ureter and gonadal vessels seen and left undisturbed. Duodenum and head of pancreas swept back. Hepatic flexure freed from gallbladder and duodenum.
Greater omentum divided up to the planned line on the transverse colon.
Terminal ileum divided about 15 cm from the ileocaecal valve and transverse colon divided at the junction of its right and middle thirds, both with a linear cutting stapler, 75 mm cartridge. Margins from tumour: about 20 cm proximal and 15 cm distal.
Both cut ends pink with good bleeding and pulsatile mesenteric edge.
Stapled side-to-side ileocolic anastomosis with linear cutting stapler 75 mm; common opening closed with a linear stapler 60 mm.
Mesenteric defect closed with continuous polyglactin 910 (Vicryl) 3-0. Omentum placed over the anastomosis.
Haemostasis checked. Peritoneal wash with warm normal saline.
Closure
Closed suction drain 16 Fr placed in the right paracolic gutter through a separate stab.
Linea alba closed by mass closure with continuous looped polydioxanone (PDS) No. 1, suture-to-wound length at least 4:1.
Skin closed with staples. Dry dressing.
Specimens and implants
Right hemicolectomy specimen (terminal ileum, caecum with appendix, ascending colon, right transverse colon, omentum) sent for HPE in formalin. Ileocolic pedicle tip marked with a long silk.
Stapler: linear cutter 75 mm, 3 cartridges; linear stapler 60 mm, 1 cartridge. Stickers pasted on the OT register.
End of procedure
Estimated blood loss 250 ml. No transfusion.
Mop, needle and instrument count correct, confirmed by scrub nurse.
No intra-operative complications.
Patient extubated and shifted to surgical ICU for observation. Pulse 90/min, BP 128/80 mmHg.
Post-op orders
Pulse, BP, SpO2 hourly for 6 hours, then 2-hourly. Hourly urine output.
NG tube on free drainage. Nil by mouth. Remove NG when bowel sounds return and flatus passed.
IV fluids and analgesia as per chart. Electrolytes and Hb tomorrow.
Drain output charted; plan removal around POD 5 if serous.
LMWH as per unit protocol from evening of surgery if no bleeding. Stockings.
Chest physiotherapy and sit out of bed on POD 1.
Follow HPE report and present at tumour board.

Must document for this operation
- Liver, peritoneum, pelvis and rest of colon findings. They decide the stage and whether further treatment is needed, and a missed second primary is a medicolegal risk.
- Level at which the ileocolic and middle colic vessels were ligated. It shows whether an oncological resection was done and helps the pathologist expect the node yield.
- Structures identified and preserved: right ureter, duodenum, SMV (superior mesenteric vein). If an injury shows up later, the note is the record that they were seen.
- Type of anastomosis with stapler models and cartridge details. Needed to manage a leak and for device traceability.
- Specimen parts, orientation marks and any separate specimens. The pathologist needs orientation to report the apical node and margins.
- Drain site and type. Ward staff chart and remove it from this line, and a leak often shows first in the drain.
Common mistakes in these notes
- Writing 'no mets' without saying the liver was palpated or seen.
- Not recording the level of vessel ligation.
- Leaving out stapler model and cartridge stickers.
- No mention of the ureter or duodenum.
- Specimen sent without orientation or with separate specimens in one container.
