Worked example

Fictional case: Mr. G.N., 45 years, farmer, primary fleshy nasal pterygium of the right eye extending 4 mm across the limbus, growing over 18 months, irritation and 2.5 D of with-the-rule astigmatism.

Case details

Date: 11/06/2026. Elective. Eye OT 2.

Block given 12:00. Incision 12:10. Pad applied 12:55.

Surgeon: Dr A. (2nd-year resident, Ophthalmology). Consultant: Dr C., present in OT.

Scrub nurse: Sr E.

Indication and pre-op

Right eye (RE) primary nasal pterygium, 4 mm onto the cornea, documented growth of 1.5 mm in 18 months, irritation, with-the-rule astigmatism 2.5 D. No suspicious features. Eye movements full.

Consent: general and procedure-specific consent taken in Kannada, including recurrence, graft loss and residual corneal scar. RE marked. WHO checklist sign in and time out done.

Anaesthesia and position

Peribulbar block, 5 ml of lignocaine 1% with bupivacaine 0.375%, no adrenaline. Aspirated before injecting.

Supine, head on ring. Conjunctival sac irrigated with 5% povidone-iodine, lids painted, draped with lashes isolated. Speculum.

Procedure

Limbus marked and pterygium transected at the limbus. Head stripped off the cornea with forceps.

Conjunctiva separated from Tenon's by sharp dissection, from 5 mm near the limbus to about 10 mm towards the semilunar fold, above and below the medial rectus.

Medial rectus drawn aside with a squint hook and protected. Tenon's capsule excised back to the caruncle. Sclera above and below the rectus checked clear of Tenon's.

Free conjunctival edge marked. Minimal trimming of the upper and lower edges. Semilunar fold excised.

Wet-field cautery to bleeding vessels near the caruncle.

Corneal bed and limbus smoothed with a diamond burr without removing corneal tissue. Faint superficial opacity remains.

Graft 13 x 14 mm marked on the superior bulbar conjunctiva, dissected from the fornix towards the limbus without taking Tenon's, leaving 1 to 2 mm of limbal conjunctiva in place.

Graft transferred to the nasal bed with the limbal edge at the limbus, marking used for orientation.

Limbal edge sutured to sclera and adjacent conjunctiva 1 to 2 mm back from the limbus with polyglactin 910 (Vicryl) 9-0. Posterior corners sutured to superficial sclera about 12 to 13 mm from the limbus. Three more sutures along upper and lower edges. Continuous suture from graft to caruncular conjunctiva to rebuild the semilunar fold. No adjuvant used.

Closure

Superior donor site left bare. Graft flat and well oriented. Pressure patch applied.

Specimens

Pterygium head with Tenon's capsule and semilunar fold sent in formalin for HPE, labelled RE nasal pterygium.

End of procedure

No intra-operative complications. Medial rectus insertion intact.

Instrument and needle count correct, confirmed by scrub nurse.

Shifted to ward. Pulse 76/min, BP 128/82 mmHg.

Post-op orders

Pad till review tomorrow morning. Do not rub the eye.

From POD 1: steroid and antibiotic eye drops as per unit regimen.

Review POD 1: graft position and colour, sutures, corneal epithelium.

Come back at once if pain, discharge or drop in vision.

Follow-up for recurrence. Repeat keratometry after 1 to 3 months before any glasses prescription.

Must document for this operation

  • Eye, side, and primary or recurrent (with number of previous operations). Recurrent pterygia need a different plan, and the count guides the next surgeon.
  • Size of the pterygium on the cornea in mm and the state of the corneal bed after removal. A residual scar is not a failure of surgery, and a written baseline shows it was there at the end of the operation.
  • Graft donor site, size and orientation. The next surgeon needs to know which conjunctiva has been used before harvesting again.
  • How the graft was fixed: suture material, number of sutures, or glue. The OPD doctor needs this to judge graft position and decide about suture removal.
  • That the rectus muscle was identified and protected. Double vision after surgery will be checked against this entry.
  • Specimen sent for HPE or discarded. A suspicious lesion that turns out to be conjunctival intraepithelial neoplasia needs histology on record.

Common mistakes in these notes

  • Writing 'pterygium excised with CAG' with no graft size, donor site or fixation details.
  • Not recording whether it was a recurrent pterygium.
  • Leaving out any adjuvant used, or writing an adjuvant without strength and time.
  • Not writing the corneal bed state, so a residual scar looks like a complication later.
  • Not advising that keratometry must be repeated before cataract surgery.
