Worked example
Fictional case: Mr. K.P., 58 years, biopsy-proven adenocarcinoma of the rectum 7 cm from the anal verge, admitted with 4 days of absolute constipation and abdominal distension. CT: distended colon up to the tumour, no perforation. Planned for neoadjuvant chemoradiation, so a diverting loop colostomy was advised.
Case details
Date: 04/05/2026. Emergency loop transverse colostomy. OT 3.
In OT 21:10. Induction 21:20. Skin incision 21:35. Stoma bag applied 22:20.
Surgeon: Dr A. (3rd-year resident, General Surgery). Assistant: Dr B. (1st-year resident). Consultant: Dr C., present in OT.
Anaesthetist: Dr D. Scrub nurse: Sr E.
Indication and pre-op
Indication: large bowel obstruction from an obstructing rectal carcinoma. Resection not planned now because neoadjuvant chemoradiation is advised first.
Stoma site marked by surgeon in right upper quadrant over the rectus, checked sitting and lying. No stoma therapist available at night.
Na 134, K 3.6, creatinine 1.1. NG tube in situ, 600 ml bilious aspirate. Foley catheter in situ.
Consent: general, procedure-specific and anaesthesia consent taken in Marathi from patient, son as witness. Explained that the stoma may become permanent.
WHO checklist sign in and time out done. Inj piperacillin-tazobactam 4.5 g IV at 21:22, before skin incision.
Anaesthesia and position
General anaesthesia with endotracheal intubation. Supine. Parts painted with povidone-iodine and draped.
Procedure
Transverse incision of about 7 cm in the right upper quadrant, midway between umbilicus and costal margin, over the right rectus.
Anterior rectus sheath incised, rectus fibres divided with cautery, superior epigastric vessels identified and ligated. Posterior sheath and peritoneum opened.
Findings: no free fluid. Transverse colon distended to about 8 cm, viable, no serosal tears. Colon identified by taeniae and appendices epiploicae.
Omentum separated from about 6 cm of proximal transverse colon between clamps and ties.
Window made in an avascular part of the mesocolon close to the bowel wall. Soft rubber catheter passed around the loop.
Loop delivered without tension, proximal limb to the right. Silastic tube 18 Fr passed through the window as a bridge.
Colon fixed to the rectus sheath with 6 interrupted seromuscular sutures of polyglactin 910 (Vicryl) 2-0. Skin closed at both ends of the incision so the loop sits snugly.
Colon opened along the taenia. Gas and liquid faeces decompressed with suction.
Stoma matured with interrupted full-thickness colon to dermis sutures of Vicryl 3-0. Both openings seen.
Closure
No separate laparotomy wound. Stoma pink, about 2 cm above skin, bridge in position.
Clear drainable stoma bag applied in OT. No drain.
End of procedure
Estimated blood loss 50 ml. No transfusion.
Mop, needle and instrument count correct, confirmed by scrub nurse.
No intra-operative complications.
Specimens: none.
Patient extubated and shifted to post-operative ward in stable condition. Pulse 92/min, BP 126/78 mmHg.
Post-op orders
Pulse, BP and stoma colour every hour for 6 hours, then 4-hourly.
Stoma output and urine output charted every shift.
NG tube on free drainage. Nil by mouth till stoma passes flatus, then sips.
IV fluids and analgesia as per chart. Urea and electrolytes tomorrow.
Bridge to be removed on POD 8 to 10.
Stoma nurse review and teaching of patient and son before discharge.
Oncology referral for neoadjuvant chemoradiation.

Must document for this operation
- Type of stoma and the segment used (ileum and distance from the ileocaecal junction, or which part of the colon). The surgeon who closes it plans the incision and anastomosis from this line.
- Which limb is proximal and how this was confirmed. A stoma matured on the wrong limb does not divert faeces, and the note is the first place checked when this happens.
- The indication for diversion and whether the stoma is meant to be temporary. It decides whether and when closure is offered, and is needed if the patient later questions a permanent stoma.
- Colour and height of the stoma at the end of surgery. Ward staff compare later checks against this baseline to pick up early necrosis or retraction.
- Rod or bridge used and the date it is to be removed. A forgotten rod ulcerates the bowel wall and makes appliance fitting difficult.
- Peritoneal findings and any biopsy taken. Turbid fluid or peritoneal deposits change the plan and the pathologist needs the site of any biopsy.
Common mistakes in these notes
- Writing 'loop colostomy done' with no segment or site named.
- Not recording how the proximal limb was identified.
- Leaving out the rod or bridge and its removal date.
- No description of stoma colour and height at the end.
- Not noting that the patient was told the stoma may be permanent.
