Worked example

Fictional case: Mrs. S.K., 38 years, P2L2, tubectomised, with HSIL on Pap smear. Colposcopy showed an acetowhite lesion entering the canal, transformation zone not fully seen. Punch biopsy: CIN 3.

Case details

Date: 08/04/2026. Elective cold-knife conisation of cervix. OT 3.

In OT 10:05. Spinal given 10:12. Procedure started 10:25. Procedure ended 10:55.

Surgeon: Dr A. (3rd-year resident, OBGY). Assistant: Dr B. (1st-year resident). Consultant: Dr C., present in OT.

Anaesthetist: Dr D. Scrub nurse: Sr E.

Indication and pre-op

Indication: CIN 3 on punch biopsy, colposcopy inadequate as the upper limit of the lesion goes into the canal. Cold-knife cone chosen over LEEP to get an endocervical specimen without diathermy artefact at the margin.

LMP 26/03/2026. Urine pregnancy test negative today. Family complete. Not on anticoagulants or antiplatelets.

Consent: general, procedure-specific and anaesthesia consent taken in Marathi from patient, husband as witness. Risks explained including bleeding up to 3 weeks, positive margins, stenosis.

WHO checklist sign in and time out done.

Anaesthesia and position

Spinal anaesthesia. Lithotomy. Vulva and vagina painted with povidone-iodine and draped. Bladder emptied with a plain catheter, urine clear.

Findings

EUA: uterus anteverted, normal size, mobile. Fornices and parametria free.

Cervix parous, healthy looking, no growth. After Lugol iodine: non-staining area from 11 to 5 o'clock on the anterior and posterior lips, going into the canal. Vagina stained normally.

Procedure

Weighted posterior speculum and anterior retractor placed.

1% lignocaine with adrenaline 1:100,000, 12 ml, infiltrated into the cervical stroma until the cervix blanched.

Stay sutures of polyglactin 910 (Vicryl) 1-0 placed at 3 and 9 o'clock, not deep or far lateral.

Circumferential incision with No. 11 blade outside the non-staining area, starting at 6 o'clock. Traction with Allis forceps. Cone made cylindrical to include the canal, apex freed with the knife. Specimen removed in one piece.

Stitch placed on the specimen at 12 o'clock for orientation. Cone measures about 2.5 cm across and 1.8 cm deep.

Endocervical curettage done above the cone apex, sent separately.

Cone bed fulgurated with ball diathermy. Monsel solution applied. Bed dry.

Closure

Stay sutures cut short and left. No vaginal pack needed. No bleeding at end. Catheter not kept.

Specimens

1. Cone of cervix, stitch at 12 o'clock, in 10% formalin, for HPE with margin status.

2. Endocervical curettings, separate container, for HPE.

Requisition forms filled with Pap, colposcopy and biopsy details.

End of procedure

Estimated blood loss 60 ml. No transfusion.

Mop, needle and instrument count correct, confirmed by scrub nurse.

No intra-operative complications.

Shifted to post-operative ward in stable condition. Pulse 80/min, BP 116/74 mmHg.

Post-op orders

Pulse, BP and pad check every 30 min for 2 hours, then 4-hourly. Inform if more than one pad soaked in an hour.

Oral fluids after 2 hours, normal diet in the evening. Mobilise when the spinal wears off.

Oral analgesic as charted.

Discharge next morning if bleeding minimal. Pelvic rest for 4 weeks. Come back at once for heavy bleeding (can occur up to 3 weeks later) or fever.

Review with HPE report in 2 weeks.

Must document for this operation

  • The Pap, colposcopy and biopsy findings that led to excision, and whether the transformation zone was fully seen. This justifies an excision rather than follow-up and explains the depth chosen.
  • Method (LEEP or cold knife), loop size or cone shape, and the approximate depth of the specimen. Depth and number of excisions are the main drivers of preterm birth risk, and the next obstetrician will ask.
  • Specimen orientation (12 o'clock stitch) and whether it came out in one piece or in several passes. The pathologist can report which margin is involved only if the specimen is oriented. Multiple pieces make margins unreliable.
  • Whether a top hat and an endocervical curettage (ECC) were done and sent separately. A positive ECC after the cone predicts residual disease and changes follow-up.
  • How haemostasis was obtained and whether a pack was left. The ward team needs to know when to remove a pack, and a late bleed is handled differently if sutures are in place.
  • Pregnancy test result. Excision in pregnancy carries a high bleeding risk and is normally deferred, so a negative test protects both patient and surgeon.

Common mistakes in these notes

  • Writing 'cone biopsy done' without the method, size or depth.
  • Not marking the 12 o'clock position on the specimen.
  • Sending the top hat or ECC in the same container as the main cone.
  • No mention of the Lugol or acetic acid findings that guided the excision margin.
  • Leaving out the post-op warning about delayed bleeding at 10 to 21 days.
