Worked example
Fictional case: Master V.M., 14 months, 10 kg, distal penile hypospadias with a hooded foreskin and mild ventral curvature. Both testes in the scrotum. Primary repair.
Case details
Date: 09/09/2026. Elective. Start 09:15, end 11:20.
Surgeon: Dr A. (MCh resident, Paediatric Surgery), under supervision. Consultant: Dr C., scrubbed for the urethroplasty. Assistant: Dr B.
Anaesthetist: Dr D. Scrub nurse: Sr E.
Indication and pre-op
Distal penile hypospadias, meatus 5 mm below the corona, ventral curvature about 20 degrees on retraction. Both testes palpable in the scrotum. No hormone treatment. Weight 10 kg.
Consent: procedure-specific consent taken from both parents in Kannada, including fistula, meatal stenosis, need for redo surgery and the stent. Parents chose circumcision rather than foreskin reconstruction.
WHO checklist sign in and time out done. Inj cefazolin at induction, weight-based dose recorded on the anaesthesia chart.
Anaesthesia and position
General anaesthesia with dorsal penile nerve block: ropivacaine 0.2%, 3 ml. Supine. Painted with povidone-iodine and draped. 3.5x loupes.
Procedure
4-0 monofilament traction stitch through the glans. 8 Fr sound passed to judge the ventral urethra.
Urethral plate marked 9 mm wide. Subcoronal incision 7 mm below the corona dorsally, inner prepuce kept for a mucosal collar, ventral incision 2 mm below the meatus. Penis degloved to the penoscrotal junction.
Findings: meatus distal penile after degloving, ventral urethra healthy. Urethral plate 9 mm wide, shallow glans groove. Glans of good size. No torsion.
Rubber band tourniquet at the base (on 09:55). Artificial erection with saline through a 25 G butterfly: residual ventral curvature about 20 degrees. One dorsal midline plication stitch of 4-0 braided polyester, knot buried. Repeat erection: straight.
Glans wings raised on both sides with iris scissors. Plate wrapped around an 8 Fr tube: tight, so a TIP was done. Midline plate incision from just inside the meatus to the end of the plate, down to the corpora.
8 Fr silicone stent passed into the bladder. Neourethra tubularised from proximal to distal with 7-0 polydioxanone (PDS), subepithelial, epithelium turned in. Suture line stopped midway between the corona and the new meatus.
Second layer: dorsal dartos flap raised, button-holed and brought over the glans onto the neourethra, tacked into the glans wings with 7-0 PDS.
Glansplasty: 5-0 PDS at the corona tied over an 8 Fr sound, glans mucosa closed with interrupted 7-0 PDS. Meatus admits 8 Fr easily, slit-shaped. Tourniquet off 10:35, total 40 min.
Mucosal collar closed with interrupted 6-0 poliglecaprone 25 (Monocryl). Byars flaps made from the dorsal prepuce. Excess skin trimmed.
Closure
Ventral skin closed in the midline with subepithelial 6-0 Monocryl, then interrupted 6-0 Monocryl.
Stent fixed to the glans with the 4-0 traction stitch, at the 12 cm mark, dripping into a double nappy.
Transparent film dressing wrapped around the penis and anchored to the scrotum and the pubic area.
End of procedure
Blood loss about 10 ml. No complications.
Mop, needle and instrument count correct.
Specimen: excess foreskin discarded. Stent: 8 Fr silicone, in situ.
Child extubated, comfortable, shifted to the paediatric surgery ward. HR 118/min, SpO2 99%.
Post-op orders
Double nappy: stent through a hole in the inner nappy, draining into the outer one. No bag.
Paracetamol and ibuprofen alternately by weight as charted. Oxybutynin if bladder spasms, by weight. Stool softener if constipated.
Remove the dressing on day 3, or earlier if it rolls down and looks tight.
Stent out on day 7 in OPD. No tub bath until then.
Petroleum jelly to the meatus and nappy area for several weeks.
Parents told to return for bleeding through the dressing, no urine in the nappy, fever or a very swollen penis.
Review after stent removal to see the stream. Long-term review after toilet training and at puberty.

Must document for this operation
- Meatus position before and after degloving. A distal meatus can turn out to be proximal once thin urethra is opened, and this decides the type of repair and the counselling.
- Degree of curvature on artificial erection, how it was corrected, and the repeat result. Residual curvature is a reason for redo surgery, and the next surgeon needs to know what was done to the corpora.
- Urethral plate width and whether it was incised. It records why TIP was chosen over another repair and is useful when a fistula or stricture appears.
- Suture material and size for the neourethra, and the coverage layer used. Fistula repair planning depends on what tissue was already used for coverage.
- Meatus calibre at the end. It is the baseline if meatal stenosis is suspected at follow-up.
- Tourniquet time. Long tourniquet times cause tissue ischaemia, and the time is checked if the repair breaks down.
- Stent type, size, fixation and planned removal day. The ward and the parents need to know how long it stays and what to do if it comes out.
Common mistakes in these notes
- Writing 'TIP done' with no meatus position or curvature.
- Not recording the artificial erection result after correction.
- Leaving out the tourniquet time.
- Not writing the stent size and the day it is to be removed.
- Not recording what was done with the foreskin and what the parents chose.
- Leaving out the block drug and volume in a small child.
Indian practice and paperwork
- Consent must be in a language the parents understand and taken by the person who will do the operation. Name who does which part when a resident operates under supervision. (NABH 5th ed., PRE.4(c) and (e))
- Paediatric records are advised to be kept until the child reaches majority, which matters here because problems can appear at puberty. (Thomas J. Indian J Urol 2009;25(3):384-8)
- Removed foreskin not sent for HPE is human anatomical waste for the yellow bag. (Bio-Medical Waste Management Rules 2016, CPCB guidelines)
