Case details

Date: 18/09/2026. Emergency list. DHS fixation, left hip. OT 4.

Injury 17/09/2026, 07:30. In OT 10:10. Spinal given 10:20. Incision 10:55. Closure completed 11:50.

Surgeon: Dr A. (3rd-year resident, Orthopaedics). Assistant: Dr B. (1st-year resident). Consultant: Dr C., present in OT.

Anaesthetist: Dr D. Scrub nurse: Sr E. Radiographer: Mr F.

Indication and pre-op

Indication: closed left intertrochanteric fracture, AO/OTA 31A1.2 (two-part, stable), lateral wall intact. Fixation within 30 hours of injury to allow early mobilisation.

X-rays: AP pelvis, cross-table lateral left hip, full left femur. Neck-shaft angle of right hip 132 degrees. No other fractures. Skin over hip, sacrum and heels intact.

Hb 11.2 g/dl. Serum sodium and creatinine normal. ECG sinus rhythm. Physician fitness given. 1 unit PRBC cross-matched.

Consent: general, procedure-specific and anaesthesia consent taken in Marathi from patient and son. Risks of cut-out, shortening, infection, DVT and death explained. Implant use explained.

Left hip marked. WHO checklist sign in and time out done. Inj cefazolin 2 g IV at 10:40, before incision.

Anaesthesia and position

Spinal anaesthesia. Supine on fracture table, padded perineal post. Right leg in well-leg holder in flexion and abduction, lateral knee padded.

Closed reduction by traction, slight abduction and 15 degrees internal rotation, left foot secured in boot. Posterior sag corrected with a padded support under the thigh.

C-arm from the right side between the legs. AP and lateral views: neck-shaft angle restored to about 135 degrees, no posterior sag, fracture gap under 2 mm.

Parts painted with chlorhexidine-alcohol and draped with an isolation drape.

Procedure

10 cm lateral incision from the vastus ridge distally. Fascia lata split in line. Vastus lateralis elevated anteriorly off the intermuscular septum, perforators coagulated, origin released from the ridge.

Guide pin entry about 2 cm below the vastus ridge, opposite the lesser trochanter. Anteversion pin placed along the anterior neck. 135 degree angled guide flush to the lateral cortex.

Guide pin passed centre-centre in the head on AP and lateral, to 5 mm short of subchondral bone. Measured length 95 mm.

Triple reamer set at 90 mm. Reaming under C-arm, guide pin did not advance. No tap needed (osteoporotic bone). Lateral wall intact after reaming.

90 mm lag screw inserted over the guide pin with centring sleeve. Wrench handle aligned with the femoral shaft.

135 degree, 3-hole side plate slid over the lag screw and tamped onto the lateral cortex. Guide pin removed. Plate fixed with 3 bicortical cortical screws (38, 40 and 40 mm).

Traction released. Compression screw inserted, fracture compressed under C-arm, compression screw left in.

Tip-apex distance 19 mm (AP 9 mm + lateral 10 mm). Final AP and lateral C-arm images saved (14 images).

Findings: two-part fracture, lesser trochanter attached, osteoporotic bone, reduction acceptable with slight valgus.

Closure

Wound washed with normal saline. No drain.

Closed in layers: vastus fascia, fascia lata, subcutaneous tissue and skin, each separately. Sterile dressing.

After release from the fracture table: left foot warm, dorsalis pedis felt. Perineum and both heels checked, no pressure marks.

Specimens and implants

No specimen.

Implants: DHS plate 135 degrees, 3-hole. Lag screw 90 mm. Compression screw. 3 cortical screws 38, 40, 40 mm. Make ______. Lot / batch numbers on stickers pasted on the implant page of the case sheet and entered in the OT implant register.

End of procedure

Estimated blood loss 250 ml. No transfusion.

Mop, needle and instrument count correct, confirmed by scrub nurse.

No intra-operative complications.

Shifted to post-operative ward in stable condition. Pulse 84/min, BP 132/80 mmHg, SpO2 98% on room air.

Post-op orders

Pulse, BP, SpO2 and urine output hourly for 6 hours, then 4-hourly.

Check left foot pulses and sensation 2-hourly for 12 hours.

AP and lateral X-ray of the left hip including the whole implant today.

Sit up in bed tonight. Out of bed to chair and walking with walker on POD 1, full weight bearing as tolerated.

Two-hourly turning, heel and sacral care.

Hb and electrolytes on POD 1. Thromboprophylaxis as per unit protocol.

First dressing on POD 2. Wound check at 2 weeks. X-rays at 2, 6 and 12 weeks.

Must document for this operation

  • Implant details: plate barrel angle, number of holes, lag screw length, screw lengths, make and lot / batch number with stickers pasted. The batch and serial number must be in the medical record for implant recall, and the next surgeon needs the make and sizes if revision is needed.
  • AO/OTA fracture type and state of the lateral wall before and after reaming. A sliding hip screw is not advised for 31A3 fractures or a broken lateral wall, so the note must show the choice was justified or what was added.
  • Tip-apex distance on the final C-arm images. A tip-apex distance under 25 mm lowers the risk of cut-out, and it is the first thing checked when fixation fails.
  • Reduction achieved and how (closed, percutaneous aids, open). Poor reduction increases failure whatever the implant, and this shows what was accepted on the table.
  • Whether the compression screw was left in. The next surgeon needs to know what hardware is inside when removing or revising it.
  • Final C-arm images saved (AP and lateral) and screening time or image count. They are the record of implant position at the end of surgery and help in audit and medicolegal review.
  • Time from injury to surgery and reason for any delay. Delay beyond 2 days is linked to higher mortality, so the reason (medical optimisation, anticoagulant) must be written.

Common mistakes in these notes

  • Writing 'DHS done' with no plate angle, number of holes or lag screw length.
  • Stickers not pasted, or pasted with no patient name on the page.
  • No mention of tip-apex distance or final C-arm views.
  • Not recording a lateral wall fracture that happened during reaming.
  • Not recording that the well leg was padded and checked after the fracture table.

Indian practice and paperwork

  • NABH requires the batch and serial number of every implant to be recorded in the patient's medical record, the master logbook and the discharge summary. Paste the implant stickers and copy the numbers into the discharge summary. (NABH Hospital Accreditation Standards 5th ed. (2020), MOM.10 objective element d)
