AI in Community Medicine / PSM / Public Health

In Indian public health, AI is used most in TB screening from chest X-rays and in scanning news for disease outbreaks. Much else, such as predicting disease risk from community data, is still at study stage. Residents should know both the uses and the limits: bias, poor data and privacy.

What AI does in Community Medicine / PSM / Public Health

AI chest X-ray screening for TBroutine

Software reads X-rays in community camps and marks people who need a confirmatory test. A 2025 study reported higher TB case yield in active case finding.

Examples: qXR (Qure.ai), CAD4TB, Lunit INSIGHT CXR

Media-based disease surveillanceroutine

A tool scans digital news across India for infectious disease reports and alerts district officers. It has run since April 2022 and issued over 4,500 alerts per press coverage.

Examples: Media Disease Surveillance tool (Government of India)

Cough-sound TB screeningearly adoption

A tool that screens for pulmonary TB in community settings. Government information reported 12-16% extra case yield compared with usual screening.

Examples: Cough Against TB

Health messaging and translationearly adoption

Chat tools draft health education material in local languages. Staff must review for accuracy.

Examples: General chat assistants

Outbreak forecastingresearch

Models use case counts, weather and mobility data to predict dengue, malaria or COVID waves.

Examples: Not listed

Risk scoring for non-communicable diseaseresearch

Models estimate diabetes or kidney disease risk from simple community data to decide who gets screened first.

Examples: Not listed

routine = in everyday use in many centres · early adoption = in use at some sites, evidence still building · research = studies only

Key ideas for this specialty

Computer-aided detection (CAD): Software that reads a chest X-ray and flags likely TB. WHO recommended it in 2021 for screening people aged 15 and above.
Syndromic surveillance: Watching early signals such as news reports, search trends or clinic complaints to spot an outbreak before lab confirmation.
Predictive model: A program that uses past data to estimate who is likely to get a disease. It is only as fair as the data it learned from.
Algorithmic bias: A tool that works well for groups in its training data and worse for others, for example rural or low-income patients.
Data governance and consent: Rules on who may collect, share and use health data, and how people agree to it. The ICMR AI guidelines cover this.

Do it yourself

Critique a surveillance AI claim

Read the TB AI-CAD field study and list its population, reference test, threshold and yield. Write a half-page on whether it would work in your district.

Open the dataset or tool →
Check the WHO CAD threshold

Use the WHO TB page and the reviewed papers to note the sensitivity and specificity targets for CAD. Compute how many false positives a camp of 1000 people would get at your local TB prevalence.

Nature Medicine: LLM decision support in Kenyan clinics improved care quality but not short-term outcomes

A cluster-randomised trial of an LLM tool ('AI Consult') inside the record system, in 16 primary care clinics with over 9,600 patients, improved documentation and treatment planning. Treatment failure was 2.2% with the tool and 2.0% without, and no harm signal was seen.

Chhattisgarh study: AI chest X-ray screening for TB raised case notifications by 80%

In Open Forum Infectious Diseases, qXR reading of 2,745 X-rays in a tribal population flagged 363 presumptive cases, 162 were confirmed, and notifications rose 80.21% against baseline. It also found 51 asymptomatic people who symptom screening would have missed.
